5th Grade Supply List

It should be understood that students are expected to have an adequate supply of paper and pencils at all time. Please check your student's supply level throughout the year!

$19.00 (make checks payable to Coopertown Middle School).
This fee provides a planner and a folder, plus art, science, and reading supplies.

Other necessary supplies:

  • Colored pencils (box of 12 colors)
  • School glue sticks
  • Crayons (box of 24)
  • Pencils (2 packs of 24 each)
  • Wide-ruled notebook paper (5 packs of 150 sheets)
    *** to be given to homeroom teacher on first full day ***
  • Folders as outlined below (plastic recommended for durability)
    • 1 folder with brads and pockets for Math
    • 1 folder with brads and pockets for Science
    • 1 folder with brads and pockets for Language
    • 1 folder with brads and pockets for Reading
    • 1 folder with brads and pockets for Social Studies
    • 1 folder with brads and pockets for Writing
    • 1 folder with brads and pockets for Spelling
    • 1 folder with brads and pockets for Health

~ No spiral notebooks, "Trapper Keepers", or rolling backpacks, please!

~ Donations of band-aids, blank white paper, and tissues (2 boxes per student) would be greatly appreciated. Additional wish list items include: staples, transparent tape rolls, jump ropes, kick balls, hula hoops, frisbees, dry-erase markers, and tennis balls (8 per student). Fifth Grade Rules and Consequences

RULES:

  1. Follow teachers' requests promptly.
  2. Be prepared with all necessary materials needed for class.
  3. Keep your hands, feet, and other objects to yourself.
  4. Treat students and adults with respect.
  5. Raise your hand for permission to speak or to leave your seat.
  6. Work quietly and do not disturb others.

CONSEQUENCES:

  1. Warning
  2. Loss of recess
  3. Note in planner
  4. Phone call
  5. Referral

** No planner or no homework results in no recess.

Grading scale

  • A: 93 - 100
  • B: 85 - 92
  • C: 75 - 84
  • D: 70 - 74
  • F: 0 - 69
